The 5th Phantom's stepmother is presumably the wife of the 4th Phantom.
Fictional Character Biography
According to Team Fantomen
A woman that the 5th Phantom referred to as his mother died in the Skull Cave in 1656. The Black King
But the 5th Phantom's mother died during his birth in 1626 Plague Infected The Ring of Death Pt 1, so this woman was presumably his stepmother.
She must have married the 4th Phantom after his affair with Giovanna in 1634 and before his death in 1646.
However, it has also been said that the 4th Phantom never remarried. The Ring of Death Pt 2
